ID Number

Any dancers who wishes to compete with Maximum Dance needs a Maximum Dance ID Number. This is FREE to do online. Fill in the box opposite and you will receive your Maximum Dance ID number.

THIS INFORMATION IS FOR OFFICIAL MAXIMUM DANCE USE ONLY AND WILL NOT BE MADE AVAILABLE TO ANY OTHER SOURCES UNLESS PRIOR PERMISSION HAS BEEN GIVEN BY THE PERSON NAMED.

Dancers need to fill in forms for which category they will compete in. All Forms must reach Maximum Dance's office 14 days prior to event day. Only one crew form per crew.
